When sciatica improves, the pain does not fade evenly. It retreats up the leg toward the spine — the foot clears first, then the calf, then the thigh. Your back may feel worse for a few days while that happens. It is the most reliable early sign treatment is working, and it is also why people abandon treatment that was working. We track it session by session so you can see it.
Pain down the leg has several common causes: a disc pressing on a nerve root, the piriformis muscle compressing the nerve behind the hip, narrowing where the nerve exits the spine, or referred pain from the sacroiliac joint that is not nerve pain at all. They can feel identical to you. They are not identical to treat — traction helps one of them and aggravates another. This is the most common reason people arrive here after weeks of physiotherapy elsewhere with nothing to show for it.
Traction where a disc is compressing the nerve, neural mobilisation to restore the nerve's ability to glide, manual therapy for the joints and muscle contributing to it, dry needling when the piriformis is the culprit, and progressive loading so the surrounding structures are strong enough to stop it recurring. You will also get specific instructions on what to avoid while the nerve settles.
